Custom Forms

Forms the installation builds for itself: private, base-wide, or public by QR code, published in versions, with every response numbered and handled.

Some forms will never be a module of their own

Lost property reports, sign-up sheets, checkout logs, and local reports each matter to someone, but none of them justifies its own program. They end up in shared spreadsheets and email, with no numbering and no one clearly handling the answers.

Custom Forms lets the installation build those forms itself, publish them to the right audience, and work every response to a status.

Build it, publish it, handle the responses

  1. Lay out the form

Place and size fields on a grid the way the dashboard is arranged. Choose from nineteen field types, including photo and file uploads and signatures, and start blank or from a template.

  2. Choose who can fill it in

Keep a form private, open it to everyone at the base, or publish it at its own address with a QR code for anyone to fill in without an account.

  3. Publish a version

Publishing freezes a numbered version, so a later edit never changes what an earlier answer meant. File a form under another module and it appears in that module’s Forms menu.

  4. Work the responses

Every response is numbered under the form’s reference code. Set its status, add a handling note, and export responses as CSV or a response as a PDF.

What it automates

Forms where the work happens

A form filed under a module opens over that module’s own pages.

Answers keep their meaning

Each response names the published version it answered.

Numbered, handled, exported

Responses are numbered, never reused, and tracked to a status.

Straight answers

Can people outside Glidepath fill in a form?

Yes, when the form is public. It gets its own address and QR code, and no account is needed. Photo and file fields stay on private and internal forms.

What happens to responses if we edit a live form?

Nothing changes for them. The builder edits a draft, people always fill in the published version, and each response names the version it answered.

Who reads the responses?

The form’s owner and the forms managers by default. The owner can share responses with everyone who can open the form.

Is Custom Forms on by default?

No. It is an opt-in module that each installation turns on.
